This remote-friendly role combines coordination, outreach, and documentation to make sure no member falls through the cracks. What you’ll do Support care coordination and follow-ups Schedule appointments and manage referrals Conduct outreach to members, families, and local providers Enter and maintain accurate documentation in the EHR Help connect members to medical, behavioral health, and community resources What you won’t do No independent billable case management Work under supervision of a Care Manager or Supervisor What matters for success Strong organization and ability to manage multiple tasks Clear, accurate documentation Professional communication with clients and providers Ability to stay calm with distressed individuals Follow HIPAA requirements in all interactions Requirements Must live in or within commuting distance of Edenton, NC High school diploma or GED Experience in healthcare, behavioral health, social services, or administrative support Basic computer skills (EHR, email, Microsoft Office or Google Workspace) Preferred Associate’s or Bachelor’s degree (Human Services, Social Work, Psychology) Experience with Medicaid populations Bilingual Work setup Remote work + local community visits Travel within the Edenton area required Reporting line Reports to Care Manager or TCM Supervisor #J-18808-Ljbffr
