Nurse Care Manager

Position Overview Navigate and coordinate the care of patients across the continuum of care. Monitor and manage resources to transition patients to the appropriate level of care in a cost‑effective manner while ensuring quality outcomes and patient satisfaction. Assess, plan, coordinate, monitor and evaluate patients’ plans of care in collaboration with the healthcare team. Monitor the patient care process and collect data to evaluate and improve care, collaborating with physicians and other healthcare workers to provide comprehensive care including health risk appraisal, health promotion and clinical outcome studies. Serve as an educator for payers, providers, and the public, and provide information regarding state and national health care trends and initiatives that enhance the provision of care and ensure efficient resource utilization. Location UHS Wilson Medical Center, Johnson City, NY; UHS Binghamton General Hospital, Binghamton, NY. The successful candidate may float across multiple UHS facilities. Work Shift and Compensation Primary work shift: Day. Compensation range: $40.70 – $61.05 per hour, depending on experience. Eligible for benefits when hired into a regular position with at least 24 scheduled weekly hours. Education & Experience Minimum: Associate’s in Applied Science in Nursing; at least 2 years of experience in an acute/tertiary facility. Preferred: Bachelor’s of Science in Nursing; at least 5 years of experience in an acute/tertiary facility. License & Certification Minimum: NYS Registered Nurse.
