JOB DESCRIPTION

In the heart of The Dalles, Oregon, Adventist Health Columbia Gorge has served the Columbia River Gorge community since 1901. We are comprised of a 49-bed hospital, various medical offices, home health, a birthing center and a vast scope of services located throughout The Dalles and the surrounding areas. The Dalles is known for its picturesque views of the Columbia River, local charm and delicious downtown restaurants and bakeries. There is something for everyone in this scenic community.

Job Summary:

Plans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives. Serves as co-leader in interdisciplinary approach to patient groups.

Job Requirements:

Education and Work Experience: Doctor of Physical Therapy (DPT) degree: Required; only if obtained license on or after January 2016; or foreign equivalent.

Licenses/Certifications: Physical Therapy licensure in all applicable states of practice: Required Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy: Required Valid Driver's License (DL) and must be at least 21 years of age or older: Required

Facility Specific License/Certifications: AHA-BLS or ARC-BLS or AHA-CPR or ARC-CPR: Required Washington-Physical Therapist (WA-PT): Required OR-PT: Required

Essential Functions: Completes evaluations to ensure quality standards and addresses assessment areas that require attention. Uses appropriate clinical techniques and tools and establishes individualized treatment plans for patients. Provides therapy interventions using appropriate treatment protocols and techniques. Establishes individualized care and discharge plans and ensures plans are communicated to appropriate parties. Completes comprehensive and accurate clinical documentation within established deadlines. Supervises assigned assistants. Performs other job-related duties as assigned.
