Clinical Nurse is a Registered Professional Nurse providing safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Delivers patient care incorporating Relationship Centered Care and nursing process based on theory, research, evidence-based practice, and organizational nursing standards. Assesses and evaluates patient care needs applying critical thinking skills. Collaborates with interdisciplinary healthcare team in plan of care development, implementation, and evaluation. Prioritizes patient care including teaching, rounding, coaching, planning after hospital care, and delegating as appropriate. Educates patients and caregivers about plan of care, transition of care, health promotion, and disease prevention. Communicates effectively and professionally with patients, family, and healthcare team. Manages assignments within Modified Primary Nursing care delivery model and allocates material resources effectively. Role models Mount Sinai Relationship Centered Care through various patient care actions such as Admission Welcome, Bedside shift Report, HELP, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. Considers patient values, preferences, cultural diversity, needs, and knowledge in care. Establishes and maintains therapeutic relationships with patients and families. Uses evidence-based practices to understand patients' perceptions of care. Initiates service recovery and escalates concerns as needed. Demonstrates caring, respect, compassion, empathy, and active listening. Supports staff use of evidence-based practices to improve patient care understanding. Conducts Purposeful Hourly Rounding addressing the 4 Ps. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety. Implements process improvement strategies based on institutional and evidence-based procedures. Engages in peer and institutional review processes. Collaborates on quality improvement projects, evidence-based practice activities, and nursing research studies. Contributes to quality and safety practices and compliance measures. Implements high reliability principles to avoid high risk behaviors and provide a safe environment. Escalates potential safety hazards following institutional chain of command. Documents nursing practice to support quality and performance improvement initiatives. Job Requirements

NYS RN License required. BLS certification required. Minimum 2 years of medicine experience. Minimum 2 years RN experience. Must have BSN degree. EPIC experience highly preferred. Must have 2 years minimum of Med Surg experience in a hospital setting. 1 year travel experience required for Travel positions, not required for Per Diem positions. Schedule Information

Schedule: 3 shifts of 12 hours each (3x12). Shift hours: 7:00 PM to 7:30 AM. Unit Specific Information

Unit is Medical-Surgical specialty. Unit census and bed count as noted above. Modified Primary Nursing care delivery model used. Focus on Relationship Centered Care and evidence-based nursing practice. Additional Details

Average unit census is 33.4 patients. 35 beds per unit.
