We are seeking a Physical Therapist to join a collaborativeskilled nursing facility (SNF) team in Petaluma, CA. This opportunity is idealfor a clinician who enjoys helping patients improve mobility, strength,balance, safety, function, and independence through individualized,evidence-based care.

Responsibilities

• Evaluate patients and develop individualizedplans of care based on functional goals.
• Provide evidence-based therapeutic exercise,mobility, balance, gait, and functional training.
• Monitor patient progress and modify treatmentplans as clinically appropriate.
• Educate patients, families, and caregivers onsafety and home programs.
• Complete accurate and timely clinicaldocumentation.
• Collaborate with physicians, nursing,rehabilitation professionals, and the interdisciplinary team.

Qualifications

• Graduate of an accredited Physical Therapyprogram.
• Active CA Physical Therapist license in goodstanding, or eligibility for licensures.
• Current BLS/CPR certification as required by theemployer.
• Strong clinical reasoning, communication,documentation, and teamwork skills.
• Experience in the applicable setting preferred;qualified clinicians at varying experience levels are encouraged to apply.
