Responsibilities
- Provide basic nursing care to residents under the supervision of a registered nurse
- Administer prescribed medications and document administration in the electronic records system
- Monitor residents’ conditions, perform ongoing assessments, and report changes promptly
- Assist residents with activities of daily living, including hygiene, mobility, nutrition, and comfort
- Maintain accurate, timely documentation and communicate effectively with the care team
- Ensure infection control practices, patient safety, and privacy standards are followed

Requirements
- Active Licensed Practical Nurse license in New York
- Experience in long-term care or geriatric care preferred
- Ability to work night shift (7:00 pm–7:00 am)
- Strong clinical assessment, documentation, and communication skills
- Ability to work collaboratively within a multidisciplinary team
- Education: Practical Nursing Diploma or equivalent
