Position Summary


The Independent Living Skills (ILS) Trainer provides individualized instruction and support to individuals with developmental disabilities and other barriers to independent living. The position focuses on teaching practical life skills that promote independence, self-sufficiency, community integration, and improved quality of life. Services are delivered in participants' homes and community settings in accordance with individualized service plans, organizational policies, and applicable state and federal regulations.


Position Scope


The Independent Living Skills Trainer is responsible for providing direct, person-centered services that enable participants to achieve identified goals related to daily living and community participation. The role requires independent work within the community while maintaining regular communication with supervisors, case managers, families, and interdisciplinary team members. The Trainer exercises sound professional judgment while ensuring participant safety, dignity, and confidentiality.


Key Responsibilities


Provide individualized Independent Living Skills (ILS) training and support to participants in home and community settings by conducting assessments, implementing person-centered service plans, and teaching essential life skills such as budgeting, meal preparation, household management, personal hygiene, transportation, community safety, communication, and social skills. Promote independence, self-determination, and community integration while monitoring participant progress, maintaining professional communication with supervisors, case managers, families, and other members of the interdisciplinary team. Accurately complete all required documentation, including assessments, progress notes, service records, and incident reports, in compliance with organizational policies, Medicaid requirements, HIPAA, and applicable state and federal regulations. Maintain participant safety, confidentiality, and professional boundaries while supporting quality improvement initiatives and performing other duties as assigned.

Confidential information shall only be disclosed as authorized by law, organizational policy, or written participant consent.


Documentation Requirements


The Independent Living Skills Trainer shall accurately complete:

• Independent Living Skills assessments
• Individualized service plans
• Daily service documentation
• Progress notes
• Incident reports
• Mileage and travel logs (when applicable)
• Timesheets
• Training records
• Communication logs
• Required electronic documentation within established deadlines

Certifications


Required:

• Current CPR Certification (or ability to obtain within required timeframe)
• First Aid Certification (or ability to obtain within required timeframe)

Preferred:

• Crisis Prevention/Intervention training
• Medication Administration certification (if applicable)
• Additional certifications related to developmental disabilities or community support services

Education


Required:

• High School Diploma or General Education Diploma (GED)

Preferred:

• Associate's or Bachelor's degree in Human Services, Psychology, Social Work, Special Education, Rehabilitation Services, or a related field.

Experience


Required:

• Experience working with individuals requiring support in community or residential settings is preferred.

Preferred:

• Minimum one (1) year of experience working with individuals with developmental disabilities, intellectual disabilities, behavioral health needs, or similar populations.
• Experience providing community-based services.
• Experience implementing individualized support plans.
• Experience documenting services within healthcare or human services environments.

Languages


Ability to read, write, and communicate effectively in English is required. Bilingual proficiency is considered an asset based on the needs of the participants served.


Driver License


Valid driver's license. Reliable personal transportation. Current automobile insurance that meets state requirements. Acceptable driving record consistent with organizational policy. Must be able to safely transport themselves between participant locations when required.


Travel Requirement


This position requires regular local travel throughout assigned service areas within South Carolina. Travel may include: Participant homes Community locations Medical appointments Grocery stores Educational facilities Employment sites OneWell Health Care offices Required meetings and training events Mileage reimbursement may be provided in accordance with organizational policy.
