As a Nurse Practitioner or Physician Assistant in Physician Services, you'll provide primary and consultative care to geriatric and medically complex patients, often in their homes or care settings, with the autonomy to focus on what matters most—delivering high-quality, patient-centered care that improves comfort, function, and quality of life. In this role, you'll conduct comprehensive medical evaluations, develop and manage individualized treatment plans, prescribe and monitor medications, and coordinate closely with physicians, caregivers, and interdisciplinary team members. Working within a collaborative practice framework, you'll serve as a key clinical decision-maker while helping patients and families better understand diagnoses, manage chronic illness, and avoid unnecessary hospitalizations. Location Office Location: Saginaw, MI Main Service Area: Houghton Lake, Roscommon, Grayling, and surrounding areas This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes Hours Monday to Friday, 8 a.m. to 5 p.m. Required Qualification Master’s or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant Current State License as a Nurse Practitioner or Physician Assistant Reliable transportation, valid driver’s license, and valid automobile insurance coverage Preferred Qualifications Minimum 2 years clinical experience in a post-acute or as a Nurse Practitioner or Physician Assistant Adult, Family, or Gerontology certification Experience with geriatric patients Essential Skills and Competencies Nursing skills as defined by accepted nursing standards Demonstrates ability to assess and respond to the needs of patients, families and caregivers in post-acute care settings Demonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles Ability to handle multiple priorities, documentation requirements and deadlines Pay Range This role provides a base annual salary (listed below) and has monthly bonus potential Pay Range $110,000 - $125,000 USD How We Care for You Minimum of 3 Weeks Paid Time Off for Full Time Part Time/Per Diem also accrue Paid Time Off Medical, Dental, and Vision Insurance HSA and FSA options including Dependent Care Company paid Short Term Disability 401(k) with Employer Match Mileage Reimbursement Pet Insurance ID and Fraud Protection And more...
