LPN - Licensed Practical Nurse

Position Details Licensed Practical Nurse (LPN) Mantey Heights Rehabilitation & Care Center Grand Junction, Colorado Pay Range: $26.00 - $38.52 per hour DOE Sign‑On Bonus: $10,000 for full‑time hires Schedule Full‑time schedule: Sunday, Monday & Tuesday, 6:00 AM – 6:30 PM PRN schedule: Flexible scheduling available based on facility needs PRN Commitment Requirements Must work a minimum of 2 shifts per month At least 1 shift per month must be worked on a weekend Must maintain active employment status and complete all required trainings and competencies What You’ll Do Provide direct nursing care to residents in accordance with physician orders and individualized care plans Administer medications and treatments safely and accurately Monitor resident conditions and report changes to physicians and nursing leadership Complete assessments and documentation in accordance with facility policies and state and federal regulations Supervise and support CNAs in delivering exceptional resident care Collaborate with physicians, therapists, and interdisciplinary team members Promote resident dignity, independence, and quality of life Qualifications Current Colorado LPN License Graduate of an accredited practical nursing program Strong clinical, organizational, and communication skills Long‑term care or skilled nursing experience preferred but not required Current CPR certification or ability to obtain upon hire Benefits Medical, Dental, and Vision Insurance Paid Time Off (PTO) Sick Leave 401(k) with company match Employee Assistance Program (EAP) Career advancement opportunities Additional Compensation Shift differentials may apply Why Mantey Heights?
