Belle is a fast-growing home healthcare engagement company seeking a compassionate Nurse Practitioner to provide remote telehealth support for Medicare beneficiaries in our Chronic Care Management program. You will assess members remotely, recommend evidence-based care, and collaborate with nurses, care coordinators, and primary care providers to keep members healthy and out of the ED when appropriate.
