- Evaluate residents' physical needs and develop tailored treatment plans.
- Implement therapeutic interventions to improve mobility, strength, and overall well-being.
- Document patient progress and adjust treatment strategies accordingly.
- Collaborate with nursing staff and other healthcare professionals to coordinate care.
- Ensure compliance with all regulatory standards and facility policies.
- Valid state licensure as a Physical Therapist.
- Proven experience in rehabilitation, preferably in senior living or memory care settings.
- Strong assessment and clinical decision-making skills.
- Excellent communication and documentation abilities.
- Ability to work independently and as part of a team.
