Location: Rockford, Illinois Start Date: ASAP Contract Length: 13 Weeks Shift: Days | Monday–Friday | 40 Hours/Week Specialty: Registered Nurse (Community-Based Care) Requirements ✔ Active Illinois RN License ✔ Minimum 3 Year of Clinical RN Experience (within the last 5 years) ✔ BSN Preferred ✔ Strong assessment, documentation, and care coordination skills ✔ Excellent communication and interpersonal abilities Responsibilities * Assess, monitor, and coordinate medical and psychiatric care for individuals served. * Collaborate with physicians, behavioral health providers, nutrition services, and interdisciplinary teams. * Coordinate healthcare appointments and ensure timely follow-up care. * Train and delegate medication administration and nursing tasks to Direct Support Professionals in accordance with state regulations and facility policies. * Maintain, review, and audit medical records to ensure accurate documentation and compliance. * Provide education and guidance to caregivers and support staff. * Participate in an every-third-week on-call rotation, primarily by phone, with support from supervisors and RN leadership.
