THIS IS A HYBRID ROLE (FIELD WORK REQUIRED). MUST RESIDE IN IL IN EITHER COOK, DUPAGE, WILL OR LAKE COUNTY FOR CONSIDERATION.
- Monday through Friday
- 8:00 AM - 5:00 PM CST
- Requires 50-75% travel for member assessments
- $35.75 per hour plus mileage reimbursement
The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual's benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes.
Job Requirements
- An active and unrestricted RN license for Illinois
- Must reside in either Cook, DuPage, Will or Lake County
- 3 years of clinical practical experience
- 2 years Care Management, discharge planning and/or home health care coordination experience
- 2 years of case management experience
- Effective computer skills including navigating multiple systems and keyboarding
- Demonstrates proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as some special proprietary applications
