**Fulltime Home Health Physical Therapist (PT) Juniata County PA**Up to a $15,000 sign on bonus**Flexible Hours**

Responsibilities: What You’ll Do

• Deliver in-home physical therapy as prescribed by physicians.
• Evaluate and treat patients with functional impairments.
• Promote independence and improved quality of life.
• Develop customized care plans and document patient progress.
• Supervise Physical Therapist Assistants and ensure care quality.
• Collaborate with the interdisciplinary team to optimize outcomes. 

Qualifications: What You Bring

• Graduate of an accredited Physical Therapy program.
• Current  Physical Therapy license in Pennsylvania .
• Passion for compassionate, patient-centered care.
• Strong communication and problem-solving skills.
• Ability to work independently and as part of a care team.
