Interdisciplinary Collaboration & Advocacy: Serve as the primary liaison between patients, families, and the care team to facilitate seamless communication and advocate for high-quality, aligned care. Clinical Monitoring & Execution: Provide high-standard clinical support by accurately assessing health status, documenting changes, and executing physician orders with precision to support the overall treatment strategy. Continuity of Care & Discharge Planning: Lead comprehensive discharge and transition efforts, empowering patients and their support networks to move confidently toward the next stage of healing. Minimum Requirements Licensure / Certification / Registration: Advanced Life Support obtained within 6 months of hire date or job transfer date. Accepted from American Heart Association or American Red Cross. BLS Provider obtained within 1 month of hire date or job transfer date. Accepted from American Heart Association or American Red Cross. Licensed Registered Nurse credentialed from Tennessee Board of Nursing or current home state license recognized under the Compact State prior to hire date or job transfer date. Education Diploma from an accredited nursing school/college or required professional licensure at time of hire.
