Health Education & Outreach Specialist

HSNT is currently seeking an energetic, mission‑minded Health Education and Community Outreach Specialist who will serve as boots on the ground, representing HSNT in the community and supporting the Marketing and Communication department in our health centres. The Health Education and Community Outreach Specialist will act as a liaison between the Marketing and Communications team and clinical teams, maintain a regular presence in HSNT’s health centres, and serve as a brand ambassador in the community. Key Responsibilities Create an outreach strategy and plan that includes regular communication with community stakeholders such as school nurses, social workers, community organizations, local universities, healthcare professionals. Partner with HSNT’s Marketing & Communication Specialist to create patient communication including targeted outreach to established patients, improve quality measures, provide health education, compose health centre signage and information about HSNT’s service lines. Support key service initiatives such as Ending the HIV Epidemic; Patient‑Centred Medical Home Diabetic Cohort; Midwives at HSNT; Pediatric Expansion through outreach, education and content creation. Identify and support outreach activities such as health fairs and community events to connect patients to care and keep HSNT top of mind. Participate in the planning and execution of stakeholder events. Engage HSNT employees regularly, identify stories to share with all stakeholder groups, strengthen the relationship between Marketing & Communications and the HSNT team. Grow HSNT’s stakeholder audience through collection of meaningful content. Participate in achieving and reporting on Marketing & Communications KPIs. Maintain a growth mindset, identifying training opportunities to strengthen required skills. Perform additional duties as assigned by the Director of Marketing and Communication. Regular in‑person attendance is required as an essential function. Work efficiently under stressful conditions and maintain timely performance. Qualifications Bachelor’s degree in health education, communication, or a related field. Minimum of 3‑5 years of community outreach, marketing, or communication experience. Proficiency with software applications including MS Office and Canva. Strong social and presentation skills, excellent verbal and written communication, skill in writing clear concise communications for print and online. Knowledge of marketing and communication principles, guidelines and best practices. Proven ability to work independently, be self‑motivated, goal oriented and self‑starter. Organizational skills: prioritize, adapt, multi‑task in changing environments. Bilingual skills (Spanish/English) a plus. Effective communication with all organisational levels. Ability to work in a collaborative, team‑oriented environment under pressure. Maintain confidentiality. Ability to relate to individuals from a variety of racial, ethnic, religious, cultural and socioeconomic backgrounds, and sexual orientations. Must be able to operate a vehicle safely during work hours. Work in‑person onsite and in the field at HSNT centres, travel within communities served and to all medical centres and outreach events.
