Your Responsibilities: * Assists with admission, transfer and discharge of residents as instructed by Charge Nurse. * Assist residents with tending to personal care and activities of daily living.

• Provide direct patient care as assigned by Charge Nurse.
• Provide proper documentation of resident care.
• Promptly communicate with Charge Nurse and leadership team with any changes

in the resident’s condition. What Success Looks Like:

• High School diploma or GED required.
• 105 hours Certified Nursing Assistant Program as approved by the Indiana

State Board of Health or equivalent as approved by the Indiana State Board of Health. * Requires having name placed on Certified Nursing Assistant Registry at time of hire as required by the Indiana State Board of Health.
