Position Description:
Case Manager: Help a young person find their next step at Rochester Youth Shelter.
180 Degrees believes every young person deserves a safe place to land, no matter what brought them to our door. Our youth shelters serve adolescents ages 12–19 navigating some of the hardest moments of their lives: homelessness, running away, family conflict, exploitation, or county placement. For many of these youth, trust, consistency, and dignity have been in short supply, and that's exactly what we're here to offer.
Every young person in our shelter needs someone in their corner, someone who sees the full picture, plans with them (not for them), and helps chart a path forward. That's you.
As a Case Manager, you provide intensive, relationship-based case management to youth during a critical window of stabilization. You're not here to control or enforce, you're here to advocate, collaborate, and build a real plan for what's next. Working closely with shelter staff and community partners, you help youth move from crisis toward stability, one conversation and one decision at a time.
What you'll actually do:
- Build trusting, relationship-based connections with youth navigating trauma and housing instability
- Lead intensive case management focused on safety, stabilization, and transition planning
- Practice trauma-responsive, youth-centered care with advocacy and collaboration, not control
- Partner closely with shelter staff to create a consistent, unified experience for every youth
- Serve as a liaison with parents, caregivers, guardians, caseworkers, probation officers, and other involved adults
- Support family and caregiver engagement and reunification efforts when appropriate and aligned with the youth's plan
- Provide regular, appropriate progress updates to families, caregivers, and workers based on youth needs, relationships, and circumstances
- Coordinate with community partners to support youth toward their next steps
- Support planned and unplanned discharges, ensuring appropriate transition planning and continuity of care.
- Maintain professional boundaries, confidentiality, and ethical practice
- Keep clear, accurate documentation to support youth planning and progress
- Lead by example, day and night by modeling trauma-responsive practice and professionalism, participate in on-call rotation, and provide expert consultation to shelter staff through complex, high-needs moments alongside your Program Manager.
This role is for you if:
You believe every young person deserves dignity, not judgment. You know that real change comes through relationship, not control. You're organized enough to manage a plan with excellent documentation, and human enough to hold space for a hard conversation. And you want a job where the work is real, human, and meaningful on every single shift.
This is a full-time, onsite position (40 hours/week). No direct reports. Direct impact every day.
Qualifications:
- Minimum age: 21.
- Bachelor’s degree in human services, psychology, social services, social work, or a related field and two years of related case management experience; case management experience may be substituted in lieu of education on a year-for-year basis.
- Experience in the nonprofit sector preferred.
- Valid driver’s license and insurance; dependable transportation for traveling to community partners and events; ability to drive Company vehicle without restrictions.
- Ability to clear a Department of Human Services background screening.
Essential Competencies:
- Strong foundation in trauma-responsive and youth-centered practice.
- Ability to build rapport with youth and families while maintaining professional boundaries.
- Skilled in care coordination, advocacy, and collaborative problem-solving.
- Understanding of confidentiality, data practices, and ethical responsibilities.
- Strong written and verbal communication skills and documentation skills.
- Excellent organization, time management, and attention to detail.
- Ability to operate effectively in fast-paced, complex environments.
- Proficiency with computer systems and databases.
Physical Demands:
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
While performing the duties of this job, the employee is regularly required to use a computer. The employee is frequently required walk, stand, and go up and down stairs. Regular computer use. Combination of seated work and movement within the shelter and community. Occasional evening, weekend, or holiday hours, including on-call responsibilities. Travel to community partners and appointments as required.
Work Environment:

Case Managers participate in the on-call rotation, and flexibility to work extended hours for events and special projects is part of showing up fully for this work. This is an on-site, awake position at our newly built, state of the art facility in Rochester, MN.
