Social Worker - Per Diem

Social Worker Per Diem Location: North Hollywood, CA. Shift: Monday‑Friday 8am–4:30pm. No evenings, no weekends. Job Overview WelbeHealth provides primary care and social services to frail seniors. The Social Worker works within an interdisciplinary team (IDT) to support participants in achieving their full potential. Essential Responsibilities Conduct psychosocial assessments to determine the needs, preferences and goals of participants and actively participate in IDT meetings to develop care plans. Deliver and document social‑work interventions as agreed upon in participants’ care plans, including arranging resources and services, assisting with care transitions, providing individual or group counseling and case management. Collaborate with the primary care physician and other team members to guide smooth transitions between settings. Initiate, coordinate and facilitate care conference meetings to ensure the highest level of care coordination among team members, participants and their support network. Provide discharge planning when participants disenroll from the program. Requirements Master’s degree in Social Work (MSW) required. Minimum one year of experience with a frail or elderly population. Experience coordinating and facilitating care conference meetings. Experience assisting people with behavioral health and substance abuse issues (preferred). Compensation Hourly rate $35.87‑$47.36, plus bonus and equity. Total rewards include 401(k) match, healthcare coverage and other benefits. Pay adjusted based on experience.
