This involves active treatment and skill development to promote growth. Create a clean, organized, welcoming home environment, collaborating with coworkers and leadership using a positive team approach. Compensation and Benefits $16.50-$18 per hour plus shift differentials. $500 sign‑on bonus for full‑time employees. Required: two weeks paid training, 9 A‑5 P M‑F. Shifts available: full‑time/part‑time. 2nd shift: 4 P‑12 A. Essential Functions Represent the organization positively in community meetings, events, and social functions. Day‑to‑day organization and implementation of activities that promote a person‑centered, organized, non‑reactive environment encouraging independence. Make independent decisions regarding best practices, quality assurance, risk management, and problem‑solving; respond to and document serious injuries. Coach, teach, and mentor individuals in all aspects of home management and personal independence, including active teaching of rights and self‑advocacy. Engage clients in ongoing active treatment per the individual support plans (ISP’s) for the ID/DD program, implementing services according to ISP and CMS regulations. Advocate for each client’s specific needs and welfare in the home and community, ensuring follow‑up on concerns. Coordinate, schedule, and/or transport clients to appointments, recreational activities, medical appointments, enrichment center, and client outings. Maintain accurate documentation of client participation and progress; ensure day‑to‑day tasks are completed. Communicate and actively participate in the inter‑disciplinary process, including ISP’s, behavior management, incident reports, etc. Interact with clients’ families and conservators, maintaining positive relationships internally and externally. Attend all required trainings and meetings and remain current on all certifications. Provide support and services to all clients across the local operation when necessary. Transport clients to medical appointments, enrichment center, and client outings as assigned. Maintain work schedules, ensuring dependability, managing time off, and accurately documenting time worked. Carry out responsibilities and duties professionally, timely, effectively, and efficiently, including other assigned job duties. Participate in additional committees or volunteer as assigned. Qualifications Job Requirements High school graduate and/or GED preferred. Valid driver’s license. CPR certification within 30 days of hire. Certified in Professional Crisis Management (PCM) within 60 days of hire. Ability to lift repetitively a minimum of 50 pounds. Competencies Good oral, written and interpersonal skills to interact effectively with others. Ability to maintain confidentiality. Ability to follow procedures, instructions, laws and policies. Ability to project a professional image on behalf of the organization. Effective teamwork and collaboration. Ability to prioritize and manage time effectively.
