Job Title: Registered Nurse

Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice Experience
License: Must have license in hand
Certifications: CPR, BLS


Must-Have:
Must be proficient in HomeCare HomeBase EMR and have used it within the last 6 months.
Must be proficient in OASIS and have used it within the last 6 months or be willing to complete a retraining course.
Must be open to traveling up to 75 miles from assigned care center.
Must be comfortable handling home health duties including driving to patient homes daily.
Must accept every patient on the schedule regardless of distance.
Must complete all documentation within 24 hours of patient appointments.
Must understand that there are no guaranteed hours with this contract.
Must meet weekly productivity points of 30.
Must be willing to participate in on-call rotations.

Description:
Coordinates and directs the provision of home nursing care according to physician's orders.
This is based on agency policies and procedures through the competent application of the nursing process.
Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses.
Home visits are required, along with periodic office meetings.
Reliable transportation and a current driver's license are necessary.
Agency-required liability insurance is also required.
Daily visits typically involve seeing 5-10 patients.
Different types of visits and activities hold varying point values.
Types of cases may include Start of Care, Wound Care, and Follow-up Visits.
Candidates must have recent specialty experience.
The average travel distance is up to 75 miles from the assigned clinic.
Mileage will be reimbursed at the IRS standard rate.
Contractor will be compensated from patient home to patient home.
The commute to and from the first and last patient is not billable.
Expectations include achieving 25-30 weekly units of productivity.
Candidates must be comfortable with travel and managing patient visits independently.
Documentation is essential for billing and follow-up patient care.
There may be backup on-call requirements depending on staffing needs.
