Description

The Workforce Case Manager provides individualized support services to participants enrolled in workforce development programs. The role involves intake, needs assessment, barrier identification, referrals, progress tracking, and consistent case management to help participants achieve sustainable employment and personal development.

Key Responsibilities:

• Conduct comprehensive assessments of participant needs, including education, skills, employment history, and personal barriers.
• Develop Individual Employment Plans (IEPs) and track progress toward milestones.
• Provide referrals to supportive services, including mental health care, transportation, housing assistance, and skills training.
• Maintain regular contact with participants to provide motivation, address challenges, and ensure program engagement.
• Coordinate with placement specialists, instructors, and external partners to support participant success.
• Maintain accurate documentation in case files and data tracking systems.
• Collect and report data on participant outcomes and program performance.
• Create action plans to remove systemic barriers, including childcare access, digital literacy, and language skills, in coordination with partner agencies.
• Coordinate with internal and external providers for mental health counseling, housing navigation, healthcare access, and emergency financial assistance.
• Facilitate or connect participants to workshops on budgeting, time management, conflict resolution, and workplace professionalism.
• Regularly assess progress against grant performance metrics (e.g., % placed in employment, % retaining housing, % completing post-secondary preparation).
• Incorporate family engagement strategies where appropriate to reinforce participants' long-term stability and success.
• Other duties as assigned.

Requirements

• Bachelor's degree in Social Work, Human Services, Counseling, or related field.
• At least 2 years of experience in case management, preferably in a workforce or social services setting.
• Strong interpersonal, organizational, and documentation skills.
• Ability to work with individuals from diverse backgrounds, including jobless, formerly incarcerated, and economically disadvantaged populations.
• Familiarity with trauma-informed care and strengths-based approaches.

Salary Description


$60,000 per year
