You’re compassionate, collaborative, and skilled in guiding individuals through customized rehabilitation plans. You’re flexible, empathetic, and bring a positive attitude that helps residents feel encouraged and empowered. Responsibilities Performs and documents an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention. Identifies and documents goals, anticipated progress, and plans for reevaluation. Plans, prepares, or carries out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients. Records prognosis, treatment, response, and progress in patient's chart or enters information into computer. Evaluates effects of treatment at various stages and adjusts treatments to achieve maximum benefit. Confers with the patient, medical practitioners, or other appropriate parties to plan, implement, or assess the intervention program. Complies with professional, ethical, and legal standards.
