This position focuses on comprehensive patient assessment, diagnosis, treatment planning, and management of chronic diseases to support patient well-being in outpatient clinics. Responsibilities

• Conduct thorough patient assessments and clinical diagnostics
• Develop and implement treatment plans tailored to patient needs
• Manage chronic diseases and provide ongoing patient education
• Oversee medication management and adjustments
• Deliver preventive care and wellness services
• Maintain accurate and timely documentation in Electronic Health Records (EHR)

Required Qualifications

• Physician Assistant License (if applicable)
• Nurse Practitioner License (if applicable)
