Responsibilities

• Provide direct and indirect skilled nursing care to adult and geriatric residents under the supervision of a registered nurse.
• Collect data on residents’ physical and psychosocial status and communicate findings to the RN and interdisciplinary team.
• Contribute to development and ongoing refinement of the resident care plan, incorporating patient and family input and coordinating with the health care team.
• Monitor resident status, reassess as needed, and initiate referrals to address all aspects of care.
• Maintain current nursing knowledge, participate in in-service programs, and assist with orientation or mentoring of staff.
• Adhere to safety, infection control, and regulatory requirements; collaborate with physicians, staff, and ancillary teams.

Requirements

• Active NY Licensed Practical Nurse license
• BLS certification (American Heart Association or RQI required)
• Minimum 1 year of LPN experience; LTC experience preferred
• Ability to work nights and to cover weekends/holidays as required by the schedule
• Demonstrated ability to care for adult and geriatric patients
• Strong communication and teamwork skills; ability to participate in an interdisciplinary care approach
