Outpatient Physical Therapist

Job Summary Evaluates and conducts medically prescribed physical therapy treatment programs to treat movement dysfunction and pain resulting from injury, disease, disability, or other health‑related conditions. Responsibilities Perform initial and ongoing assessments of patient condition. Establish, revise, and evaluate a plan of care appropriate to problems identified, involving patient and family. Perform therapy interventions utilizing standard physical therapy techniques and skills as appropriate for patient condition. Provide education to patient/family/staff by assessing educational needs. Participate in discharge planning and maintain continuity of care, providing home instructions, recommending equipment/home modifications, and appropriate follow‑up care. Oversee activities of physical therapy assistants, students, and other support personnel. Maintain documentation detailing patient response and need for skilled intervention per regulatory bodies. Work in a constant state of alertness and safe manner. Perform other duties as assigned. Education & Qualifications Bachelor’s, Master’s, or Doctorate degree in Physical Therapy, or related field required to obtain licensure in the state of practice. No experience required. Physical Requirements Constant standing; frequent lifting/carrying/pushing/pulling items weighing 0–25 lb. Frequent use of hearing, speech, vision (near 20 inches or less, far 20 feet or more). Frequent walking, reaching, gripping, and keyboard use/data entry. Occasional bending, stooping, climbing, kneeling, squatting, twisting, repetitive foot/leg and hand/arm movements. Occasional lifting/carrying/pushing/pulling items weighing 25–50 lb. Occasional lifting/moving patients. Occasional driving; rare crawling. Professional Licensure & Certifications Resuscitation Certification: Basic Life Support (BLS HCP) required within 90 days. Other resuscitation certifications may be required depending on department. State licensure needed: Illinois, Missouri, Oklahoma, Wisconsin as applicable. Work Shift & Type Day Shift. Employee – Regular.
