Job Description Summary UofL Health is hiring LPNs for PCU, psych, and rehab units across our system. Part‑ and full‑time available. Days and nights available. Locations Frazier Rehab Jewish Hospital Mary and Elizabeth Hospital Peace Hospital UofL Hospital Position Summary and Purpose The LPN is responsible for providing patient care utilizing the nursing process. The nurse collaborates with members of the interdisciplinary team and ancillary staff to provide effective, safe quality care. The LPN is a key member of a team that promotes patients’ recovery and functional improvement. Essential Functions Demonstrates knowledge of nursing theory and practice within the scope defined for LPNs by the Kentucky Board of Nursing. Completes a comprehensive daily assessment on assigned patients as evidenced by daily shift report and documentation in the patient’s record. Prioritizes care based on nursing assessment of patient needs. Reviews and revises patient care plans according to established policy and procedure. Delivers direct patient care based on physicians’ orders and nursing interventions established on patient care plan. Documents interventions and progress toward patient goals. Monitors laboratory values and reports abnormalities within 30 minutes of critical results. Administers and documents prescribed medications (scheduled/PRN) according to schedule and procedure. Initiates nursing care by securing and implementing physicians’ orders without direct supervisor instruction. Reviews all MD orders each shift and implements them. Assists in developing and delivering patient education to meet individual goals. Communicates effectively with interdisciplinary team members. Reports patient information/status to nurse in charge and immediate supervisor as patient condition changes. Participates in chart rounds and provides positive communication to patient families. Escalates unresolved issues to appropriate personnel in a timely manner. Demonstrates appropriate skills for age‑specific medication dosing, including geriatric, adolescent, and pediatric practices. Calculates dosage adjustments for geriatric patients based on kidney function. Other Functions Completes special projects as assigned. Maintains patient care areas neat and clean. Labels patient supplies per policy and procedure. Ensures equipment functions properly during patient use. Follows universal precautions for all patients. Plans for patient safety using safety standards. Contributes to performance improvement activities. Wears name tag and locator badge at all times. Attends shift meetings and reads all minutes. Applies current nursing literature to clinical situations. Sets and meets annual professional development goals. Completes annual competencies within defined timelines. Maintains compliance with employee health requirements. Completes all mandatory modules/in‑services annually. Provides evidence of completed educational programs to the Education Department. Ensures HIPAA privacy and security compliance. Performs other duties as assigned. Education Completion of an LPN program at an accredited school of nursing (required). For LPNs graduating prior to September 15, 2019, completion of a Kentucky Board of Nursing approved IV course is required (excluding UofL Physicians and Peace Hospital). Experience Prior acute care or rehab experience (preferred). Licensure Current license to practice as an LPN in the Commonwealth of Kentucky (required). Certification CPR/BLS required upon hire (American Heart Association) (required). Job Competency Knowledge of human injuries, diseases, and treatments including drug properties and interactions.
