Hospice Home Health Aide (LNA) – Bring Comfort, Dignity, and Compassion Into the Home

As a Hospice Home Health Aide (LNA), you’ll provide compassionate personal care and support to hospice patients in their homes, helping promote comfort, dignity, safety, and quality of life. You’ll work closely with nurses, therapists, patients, families, and other members of the interdisciplinary care team while following each patient’s established plan of care.
When appropriately certified and trained, you’ll also have the opportunity to provide Reiki/touch therapy services as part of the patient’s plan of care.

- Reiki Certification Support: Already Reiki certified? Put those skills to work supporting patient comfort. Not yet certified? We welcome candidates willing to obtain certification within six months of employment.
- Ongoing Training: Receive orientation, competency development, continuing education, and support from an experienced clinical team.
- Collaborative Care: Work alongside nurses, therapists, patients, families, and other members of the interdisciplinary hospice team.
- Independent, Home-Based Care: Provide individualized care in patients’ homes while working within an established plan of care and scope of practice.
- Provide personal care, including bathing, grooming, dressing, toileting, and other activities of daily living.
- Promote patient comfort, dignity, independence, and participation in self-care whenever possible.
- Assist with mobility, ambulation, positioning, transfers, and prescribed exercise activities.
- Assist with self-administered medications as directed by the patient’s plan of care.
- Support patients with meals, hydration, and other nutrition-related needs.
- Provide Reiki/touch therapy when indicated in the plan of care and appropriately certified and trained.
- Perform light housekeeping and homemaking tasks necessary to maintain a safe and healthy home environment.
- Perform simple procedures as an extension of physical, occupational, or speech therapy when directed and supervised by the appropriate therapist.
- Observe patients for changes in condition, comfort, safety, needs, or functional status and promptly report concerns to the appropriate clinical team member.
- Accurately document care provided and patient observations according to agency standards and timelines.
- Communicate professionally with patients, families, supervisors, and members of the interdisciplinary care team.
- Follow infection control, safety, confidentiality, compliance, and professional practice requirements.
- Participate in required orientation, competency assessments, continuing education, and in-service training.
Required
- Successful completion of an approved Licensed Nursing Assistant (LNA) training program meeting applicable Medicare and State requirements;
- Current New Hampshire LNA license/registration ;
- Current CPR certification;
- Successful completion of required agency competency evaluations;
- Reiki certification or willingness to obtain Reiki certification within six (6) months of employment;
- Completion of, or willingness to complete, required hospice and/or end-of-life training and annual competencies;
- Ability to complete at least twelve (12) hours of nursing-related in-service education annually;
- Valid driver’s license;
- Reliable automobile meeting New Hampshire registration and applicable automobile insurance requirements; and
- Ability to travel to patient homes throughout the assigned service area.
Preferred
- At least one year of active employment experience as an LNA;
- At least six months of experience in a nursing home, hospital, home health, hospice, or similar healthcare setting; and
- Previous hospice or end-of-life care experience or training.
