Licensed Clinical Social Worker
Job Summary:
Avir at Lufkin, a prominent healthcare organization, is seeking a highly skilled and compassionate Licensed Clinical Social Worker to join our team. Responsibilities:
Conduct individual and group therapy sessions with patients to address mental health needs
Develop and implement comprehensive treatment plans, incorporating patient-centered care principles
Collaborate with healthcare providers and social workers to ensure seamless care coordination
Provide crisis intervention and stabilization services as needed
Document patient progress and maintain accurate records
Participate in team meetings and contribute to continuous quality improvement initiatives
Requirements:
