Locations Providence Medford Medical Center(HR) (On-site) Job Schedule Full time FTE 1 Minimum Salary 44.18 Maximum Salary 68.57 Job Description Manages and provides skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician plan of treatment, taking into account individual patient special or age related needs. Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individuals functional level and potential. Required Qualifications: Bachelor's Degree from an accredited Physical Therapy Program. or Master's Degree from an accredited Physical Therapy Program. or D.P.T. from an accredited Physical Therapy Program. National Provider BLS - American Heart Associationwithin 30 days of hire. Oregon Physical Therapist Licenseupon hire. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
