Inpatient Social Worker

Schedule Full-time, 40 hours/week 1st shift, 7am-4pm/8am-4:30pm Rotating weekends and rotating holidays Location Baystate Medical Center Springfield, MA Job Specific Duties / Responsibilities Provides a comprehensive age-specific, psychosocial assessment of patients, including social, psychological, cultural, environmental and financial information, and utilizes this information to develop a psychosocial treatment plan. Incorporates cultural and spiritual variables into the assessment. Assesses the risk of abuse/neglect for children, disabled adults and/or elders as appropriate. Files reports with appropriate agencies as needed. Provides counseling services to patients and families. Utilizes diverse counseling techniques to help patients and their families deal with their medical situations more effectively. Remains current in social work practice trends. Demonstrates strong social work skills. Participates as part of the care management team using discharge planning knowledge and skills to ensure that appropriate post-hospital care plans are developed and implemented in a timely manner. Coordinates and documents transition of care plan toward the desired outcome, making recommendations and revision to the plan as indicated. Proactively evaluates the psychosocial and physical needs of the patient and family, insuring appropriate transition of care. Make referrals as needed to social service/community agencies or programs. Communicates and involves the health care team, patient and family throughout performance of this process. Leads or contributes to new initiatives within the department or in the hospital as assigned. Minimum Qualifications Master's Degree in Social Work required Minimum of three years as a practicing social worker in a health care or social service setting (may include some time in an equivalent Master’s level internship setting) preferred LICSW or LCSW with Master's degree also be considered Education: Masters Degree (Required) Certifications: Licensed Independent Clinical Social Worker - State of MassachusettsState of MassachusettsState of Massachusetts Advantages of Working With Baystate! Typically, a new hire can expect a starting salary somewhere in the lower part of the range. Actual salaries may vary by position and will be determined based on the candidate's relevant experience. No employee will be paid below the minimum of the range. Pay ranges are listed as hourly for non-exempt employees and based on assumed full time commitment for exempt employees.
