Join Our Growing Wound Care Team

We are seeking a compassionate, detail‐oriented In‐Person Licensed Practical Nurse (LPN) to support our wound care providers across skilled nursing facilities and other clinical settings. This hands‐on role is essential in ensuring high‐quality wound care delivery, accurate clinical documentation, and seamless coordination with facility staff.

This is an excellent opportunity for LPNs interested in wound care, vascular medicine, or advancing their clinical skills in a specialized field.

Key Responsibilities

• Assist wound care providers during on‐site patient visits
• Perform and document detailed wound assessments, including:
• Wound measurements
• Tissue type and wound characteristics
• Drainage and periwound condition

Dressing changes and wound photography (if applicable)

• Prepare patients and treatment areas for procedures such as debridement and advanced wound therapies
• Carry out provider‐directed wound care treatments and dressing applications
• Accurately document clinical findings and treatment plans in the EMR
• Communicate effectively with facility nursing staff to coordinate care
• Maintain organized, up‐to‐date patient charts and ensure documentation accuracy
• Support providers by anticipating clinical needs and maintaining efficient workflow
• Follow all HIPAA and patient confidentiality standards

Qualifications

Required

• Active LPN license (state‐specific)
• Strong clinical assessment skills
• Experience with wound care or general nursing in SNF, home care, or outpatient settings
• Excellent attention to detail and documentation accuracy
• Ability to work independently and travel to assigned facilities
• Professional, reliable, and patient‐focused

Preferred

• Prior experience in wound care, vascular care, dermatology, or surgical nursing
• Familiarity with EMR systems (PointClickCare, Athena, eClinicalWorks, Epic, etc.)
• Understanding of wound staging, measurements, and documentation standards
