Clinical Nurse Consultant II (CNC II) for Cancer Prevention and Genetics

The Clinical Nurse Consultant II (CNC II) for Cancer Prevention and Genetics is accountable for facilitating the delivery of patient care utilizing the Nursing Process and Relationship-Based Care model as the framework. The CNC II provides patient care and consultation, coordinates the educational development of staff members, and acts as a role model in clinical practice. Through shared leadership, Evidence-Based Practice, Relationship-Based Care, and standards of nursing excellence, the CNC II works to improve patient outcomes while supporting unit, clinic, and organizational goals. The CNC II serves as a clinical expert, educator, consultant, and change agent for cancer prevention and genetics across primary care clinics. The CNC II collaborates with primary care providers, nurses, care managers, genetic counselors, specialists, and other members of the multidisciplinary team to identify patients at increased hereditary cancer risk, facilitate appropriate genetic evaluation and testing, promote risk-appropriate cancer screening, and support patients and families in understanding and managing hereditary cancer risk. A key responsibility of this position is the implementation, education, optimization, and sustainability of NEST Genomics, which integrates clinical decision support and care management into primary care workflows, making family cancer history actionable and supporting timely identification, evaluation, and management of patients at increased hereditary cancer risk.

Duties & Responsibilities

• Assists the nursing staff in a clinical service to develop effective nursing care plans by conducting studies of existing patient care programs, identifying deficiencies, and developing appropriate remedial educational programs.
• Support Implementation of NEST Genomics across designated Primary care clinics.
• Partner with primary care leadership, nursing, providers, care management, informatics, information technology, genetic counseling, and other stakeholders to integrate NEST into existing clinical workflows
• Serve as a clinical nursing expert and resource for cancer prevention, hereditary cancer risk assessment, genetic testing, and risk management within primary care. Participates in the planning and coordination of nursing and medical assistant activities.
• Analyzes and outlines factors for the achievement of effective performance in Nest Integration and other workflows related to Cancer Prevention and Genetics.
• Collaborates with, supports, and participates in departmental and organizational education and initiatives through Clinical Practice and Professional Development.
• Initiates, conducts, and analyzes evidence-based practice research to improve patient care outcomes and supports other nursing and multidisciplinary research efforts on the unit and across UI Health.
• Promotes early identification of patients who may be at increased risk for hereditary cancer based on personal and family cancer history.
• Review and assess relevant personal and family cancer history to support appropriate hereditary cancer risk assessment.
• Collaborate with primary care providers and multidisciplinary teams to determine appropriate clinical follow-up for patients identified as having potential hereditary cancer risk.
• Support care coordination and follow-up for patients requiring additional evaluation, genetic testing, enhanced surveillance, or risk-reduction interventions.
• Provide patient-centered education regarding hereditary cancer risk, genetic testing, cancer prevention, screening, and risk-reduction strategies.
• Quality Improvement: Participate in or lead projects focused on cancer prevention and hereditary cancer risk assessment. Collects, monitors, and analyzes program data related to screening outcomes, imaging follow-ups, and clinical performance indicators.
• Organizational: Adheres to UI Health Code of Conduct Treats patients /families, staff, students, and visitors with respect and dignity. Completes electronic learning modules and related programs assigned by UI Health. Complies with University Health Services TB and Fit Testing.
• Education and Research: Collaborate with other nurses and physicians to identify, promote, and provide positive learning experiences. Contribute to internal and external reporting, presentations, and quality improvement initiatives tied to program metrics, research, and grant activities. Serve as a mentor, educator, and clinical resource for nurses and other members of the healthcare team. Take responsibility for continued professional growth /development by identifying learning needs and initiating strategies to meet such needs.

Minimum Qualifications

1. BSN (or higher) required of an accredited nursing program.
2. Eligibility for registration with the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation. Out of state and compact licenses are not accepted.
3. Two (2) years of nursing experience in the particular branch of service to which assigned, one year of which involved work comparable to a Clinical Nursing Consultant I.
4. Ability to select appropriate teaching methods based upon the educational and experience backgrounds of employees.
5. Current AHA BLS/CPR issued by the American Heart Association (AHA).
6. Current ACLS issued by the American Heart Association (AHA), or within 6 months of hire for main hospital.
7. Nationally certified through ANCC approved certifier, or within 6 months of hire in an area relevant to their clinical area.
8. Effective communication skills (oral and written) that facilitate therapeutic relationships between patients, family, and health care providers
9. Intermediate computer skills with the ability to become proficient in the Electronic Health Record (EHR) with on-the-job training
10. Excellent attendance, interpersonal skills and work history required.

Preferred Qualifications Two (2) years of outpatient ambulatory nursing experience in an academic medical center.
