Clinical Liaison - Behavioral Health | SNF/LTC

We are seeking a Clinical Liaison to support census growth and referral development for a skilled nursing facility with a strong behavioral health population . This role is ideal for someone who understands both the clinical and relationship side of admissions and thrives in a fast-paced environment. Responsibilities Manage incoming referrals and evaluate clinical appropriateness for admission Build and maintain relationships with hospitals, behavioral health facilities, and community partners Serve as the primary point of contact for referral sources and coordinate admissions Work closely with internal teams to ensure smooth transitions and timely admits Support census growth through consistent follow-up and outreach Navigate complex behavioral and high-acuity cases with confidence Track referral activity, conversions, and opportunities for growth Qualifications Experience in SNF, LTC, or behavioral health admissions/liaison role required Strong behavioral health experience highly preferred Clinical background (RN, LPN, or strong clinical knowledge) preferred Proven ability to build relationships and drive admissions Strong understanding of insurance, referrals, and placement processes Ability to manage multiple referrals and move quickly Starting salary $85,000+ (based on experience) Performance-based bonus tied to census/admissions Full benefits package #J-18808-Ljbffr
