Care For the Homeless is seeking a Case Manager to provide direct case management services for individuals experiencing homelessness, addressing social, mental health, substance use and medical needs to promote stability and empowerment. The role involves intake, assessments, collaboration with multidisciplinary teams, and advocacy for entitlements and services.
