The ideal candidate brings both clinical competency and a genuine heart for serving vulnerable populations with dignity, respect, and professionalism.Key ResponsibilitiesAdminister prescribed medications and treatments accurately and in accordance with established policies and proceduresMonitor and document resident vital signs, health changes, and responses to treatment on an ongoing basisPerform wound care, catheter care, and other skilled nursing procedures as directed by the care planCommunicate resident status updates clearly and promptly to the supervising registered nurse and attending physicianAssist in developing, reviewing, and updating individualized resident care plans in collaboration with the interdisciplinary teamRespond promptly and calmly to resident needs, emergencies, and changes in conditionProvide emotional support and education to residents and their family membersSupervise and delegate appropriate tasks to certified nursing assistants while ensuring quality care deliveryMaintain accurate, timely, and thorough documentation in electronic health recordsFollow all infection control, safety, and regulatory compliance standardsRequirementsCurrent and valid Connecticut LPN license in good standingGraduation from an accredited Licensed Practical Nurse programMinimum of one year of clinical nursing experience, preferably in a long-term care, skilled nursing, or assisted living environmentDemonstrated ability to work effectively within a multidisciplinary team settingStrong written and verbal communication skillsProficiency with electronic health record systems or willingness to learn quicklyAbility to remain composed, organized, and compassionate under pressureReliable attendance and a strong commitment to professional accountabilityCPR and Basic Life Support certification required or must be obtained prior to start datePreferred QualificationsExperience in a related field is preferredScreening QuestionsHow many years of experience do you have relevant to this LPN - Licensed Practical Nurse role?Do you hold any required licenses or certifications for this position?Are you available to work onsite in Wallingford, CT?
