Independent Primary Care NP/PA - Home & Community Medicine

Bloom Hospice

Bloom Healthcare in Castle Rock, CO is seeking a Primary Care NP/PA to deliver patient-centered, home- and community-based primary care. You’ll manage a defined panel, address chronic and acute needs, and coordinate with an interdisciplinary team using Bloom’s data systems to improve outcomes. This role offers a manageable schedule, meaningful autonomy, and robust benefits.
