Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient setting
• Evaluate, assess, and develop individualized treatment plans for patients
• Deliver hands-on therapeutic interventions in alignment with plan of care goals
• Maintain accurate and timely clinical documentation in accordance with facility requirements
• Collaborate with interdisciplinary team members to support patient progress and outcomes

Required Qualifications

• Active Physical Therapist license required
• Experience in an outpatient physical therapy setting
• Ability to meet credentialing and contract quality assurance requirements prior to start
• Must obtain a permanent Delaware state license; compact licensure may be accepted at start but permanent licensure must be obtained promptly

Preferred Experience / Skills

• Prior experience working in a high-volume outpatient therapy environment
• Familiarity with electronic medical records and external documentation platforms
