- Bachelor's degree and relevant certifications are required. - Required to work one weekend per month. - Must provide a Driver's License/State ID upon submission. - Two professional references are required for submission. - An Ascension Profile is required for submission. - Professional:Ascension Employment Verification is required for onboarding. - Ascension Skills Checklist is needed for onboarding. - Personal Health History including MMR, Fit Test, Hepatitis B, Health Statement, 2 Step PPD OR IGRA, Varicella, and Tdap are required for onboarding. - Ascension Background Check Results are required for onboarding. - An Ascension 12 Panel Drug Screen is required for onboarding. - Ascension Confirmation of Assignment is required for onboarding. - Education Verification by Ascension is required for onboarding.
