• Possibility of June coverage, 2 weeks prior to stat.
• 2-3 weekends per month ongoing.

Submission Requirements

• Board Certified [REQUIRED]
• OH license (willing to accept providers needing to obtain State License) [HIGHLY PREFERRED] - must be OH licenses for June/July start.
• BLS & ACLS [REQUIRED]
• Active DEA [REQUIRED]
• Clean Malpractice [REQUIRED] Emergency temps can be obtained
• A Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date. [REQUIRED]
