Therapist - Physical Therapy Assistant

Job Summary:

We are seeking a dedicated Physical Therapy Assistant to provide home health therapy services, including home visits and patient follow-ups. The ideal candidate will support patient rehabilitation goals while ensuring quality care and compliance with industry standards.

Responsibilities:

Deliver skilled physical therapy services to patients in their homes, following physician's plans of care.
Conduct initial assessments, treatment planning, and follow-up visits to monitor patient progress.
Document clinical activities accurately in the EMR system within 24 hours of each visit.
Communicate with supervising therapists, case managers, and patients to coordinate care effectively.
Maintain reliable transportation and ensure all documentation, travel, and activity points meet agency requirements.
Adhere to patient scheduling, productivity expectations, and on-call schedules as required.

Qualifications:

Hold an active and valid state license as a Physical Therapy Assistant.
Have at least 1 full year of recent home health experience.
Possess current CPR and BLS certifications.
Demonstrate proficiency with home health EMR systems, preferably HomeCare HomeBase (HCHB).
Be willing to travel up to 75 miles from the home clinic and accept patient schedules regardless of distance.
Maintain reliable transportation, driver's license, and agency-required liability insurance.
