Licensed Clinical Social WorkerSalvo Health is looking for an experienced Licensed Clinical Social Worker to play an integral role on our care team. This role will work directly with patients virtually, offering personalized psychosocial support and counseling to patients to reduce symptoms and improve quality of life. The LCSW will empower patients with the knowledge and tools to make lasting behavioral changes that will improve clinical outcomes and overall wellness. You'll serve as one of our patients' primary success partners, with responsibilities including: addressing member health questions, providing relevant educational resources, and fostering positive member accountability. If you have a passion for patient empowerment through a holistic-based approach to medicine, come join our team!Responsibilities:Provide case consultation and collaborate with interdisciplinary care teams.Conduct case management, including assessing patient needs, coordinating care, and connecting patients with community resources.Lead group sessions focused on psychoeducation, stress management, mindfulness, and meditation.Conduct one-on-one patient visits to provide emotional support, stress management techniques, psychoeducation, and coping strategies.Document patient encounters and coordinating follow-up care to support positive health outcomes.Requirements:LCSW license with licensure to practice in GA, FL, and MI (also open to PA, NY, OH)2+ years experience as an LCSWBachelor's and Master's degrees requiredExperience communicating with patients asynchronously preferredExperience with GI conditions or weight management a plusProficiency in using telehealth platforms and electronic health record (EHR) systemsPassion for working with clients to facilitate behavior change by helping them set health goals to effectively manage chronic diseaseExperience working with patients in a clinic setting preferredDemonstrates a willingness to go above and beyond normal responsibilities to provide the best care possibleExcellent interpersonal communication and problem-solving skillsExceptional organizational skillsExperience and ability to thrive in a fast-paced, high-growth environmentStrong personal computing skills - remote work experience preferredPassion for and commitment to functional medicine approachesSalvo Health is taking a new approach to help millions of Americans facing chronic health conditions, centered on chronic gut health and metabolic conditions from IBS to obesity.
