Nurse Practitioner or Physician Assistant - Part Time Weekend Rounding (Fri-Sun)

Summary This part-time opportunity is ideal for an experienced NP or PA seeking a consistent weekend schedule with meaningful patient impact. In this role, you will provide weekend rounding coverage at a long‑term care/skilled nursing facility, seeing 15‑20 residents each Friday, Saturday, and Sunday. You will conduct comprehensive assessments, manage acute changes in condition, coordinate with facility nursing staff, and ensure continuity of care for a stable, established patient panel. No weekday commitment required. Essential Duties & Responsibilities Patient Care and Assessment: Perform comprehensive assessments, develop treatment plans, and provide ongoing care management for residents focusing on preventive care and chronic disease management. Routine Rounds and Monitoring: Conduct regular rounds to assess residents’ conditions, adjust treatment plans, and address emergent health concerns. Care Coordination: Collaborate with facility staff, family members, and other healthcare providers to ensure comprehensive, coordinated care tailored to the residents’ needs. Patient Education: Educate residents, families, and facility staff on disease prevention, wellness, and managing chronic illnesses to support better health outcomes. Documentation and Compliance: Maintain accurate, timely, and thorough documentation within the electronic medical record (EMR) system, ensuring compliance with regulatory standards and organizational protocols. Quality Assurance: Participate in quality improvement initiatives to enhance care delivery, reduce hospital readmissions, and optimize patient satisfaction within the facility. This role requires travel to and from one or more assigned facilities. Depending on the amount of travel required, Curana may in future determine that a valid driver’s license, current auto insurance, and an acceptable driving record are required.
