Join a growing primary care practice based in Williamsburg providing telehealth visits for patients in assisted living facilities. This role offers a stable, supportive environment and the opportunity to build ongoing relationships with a recurring patient panel. (Not a remote position.)
Responsibilities:
- Provide comprehensive primary care services to geriatric patients via telehealth.
- Conduct virtual visits with medical assistant support at each assisted living facility.
- Review and interpret lab results and diagnostic data collected onsite.
- Maintain accurate and timely documentation using established templates.
- Collaborate with other PAs, NPs, and medical staff to ensure continuity of care.
- Deliver high-quality, compassionate care to patients within an organized telehealth system.
Qualifications:
- Licensed PA or NP in New York State.
- Credentialed with New York State Medicaid and Medicare (preferred).
- Experience in primary care, telehealth, or geriatrics preferred but not required.
- Strong clinical judgment and ability to work independently.
