Advanced Practice Provider

This is an Advanced Practice Professional (APP) opening for either a Nurse Practitioner (NP) or Physician Assistant (PA) in the Medicine Pulmonary service. One full‑time position (1.0 FTE) will be filled. Job Summary The Medicine Pulmonary service is a team of advanced practice providers (APPs) that care for patients with complex medical needs at general, moderate, and ventilator care levels. The service serves pulmonary transplant patients, cystic fibrosis patients, post‑operative interventional pulmonary patients, and adult patients requiring chronic invasive and non‑invasive ventilation. APPs deliver high‑quality evidence‑based care under the supervision of a pulmonary critical care attending. Responsibilities Perform initial assessments of ventilator/moderate care patients and general care patients. Perform detailed history and physical, formulate initial assessment and plan, and present patient cases and any pertinent data to medical staff. Provide continuing care and discharge management of patients on the service. Appropriately order, collect, and interpret reports from diagnostic lab testing, radiographic procedures, and other clinical studies. Prescribe medications and enter other orders related to patient care. Perform diagnostic, therapeutic, and procedural interventions within privileges, such as chest tube removal/change and tracheostomy management. Manage mechanical ventilation, non‑invasive ventilation, heated high‑flow nasal cannula for chronic and acute on chronic respiratory failure within unit guidelines. Manage antibiotics and antifungals. Manage hemodynamically stable gastrointestinal bleeding and liver failure patients. Manage renal failure patients requiring dialysis. Manage immunosuppression and opportunistic infections in transplant patients. Run ACLS codes for cardiac arrest. Initiate transplant evaluations for end‑stage lung and end‑stage liver patients. Assist attending physicians with orientation of new medical trainees and training of new APPs; maintain continuity of care. Communicate appropriately with attending physicians, consultants, and all patient‑care team members to coordinate care. Maintain accurate documentation of patient status, evaluations, and treatments. Streamline patient transfer processes between levels of care, including discharge orders, summaries, and communication. Participate in quality assurance activities, including quality improvement projects, peer evaluations, and research initiatives. Assist in developing patient‑specific, facility‑based disease‑management protocols and utilize new technologies as appropriate. Organizational Relationships and Practice Environment APPs receive medical supervision from the Medicine Pulmonary Service Chief. APPs receive administrative supervision from the APP Manager. Required Qualifications – Nurse Practitioner (NP) Current licensure to practice as a Registered Nurse with NP specialty certification in Michigan. Graduate of an accredited Nurse Practitioner program. Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN). Current specialty certification to practice as an Adult Gerontology Acute Care Nurse Practitioner. Required Qualifications – Physician Assistant (PA) Current license to practice as a Physician Assistant in Michigan. Completion of an accredited Physician Assistant program (ACGME or equivalent). Certification by the National Commission on Certification for Physician Assistants (NCCPA). State of Michigan Controlled Substance License. Current DEA registration. ACLS certification (AHA approved); BLS required unless ACLS completed through UMHS Simulation Center. Experience rounding on inpatient service as a credentialed APP caring for acutely ill patients. Desired Qualifications At least 8 months of advance practice experience. Clinical experience with mechanical ventilation, non‑invasive ventilation, heated high‑flow nasal cannula for chronic and acute on chronic respiratory failure. Demonstrated ability to coordinate clinical care across services and communicate as a patient advocate. Ability to prioritize assignments and manage a busy work environment. Benefits Full‑time salary range: $130,630 – $200,734 per year.
