Mental Health Case Manager: Community Resources & Housing

FriendHealth

Friend Health in Chicago is seeking a qualified candidate to provide direct services to individuals as specified in their treatment plans. The role requires close collaboration with State Operated Mental Health Facilities and community support teams to develop plans for treatment and resources. Ideal candidates will have a Bachelor's degree in a related field and a valid Driver's License. This position involves moderate physical activity and requires knowledge of social services and medical needs for persons with mental disabilities.
