Type of Opportunity: Full time Job Exempt: Yes Job is based: Reverend Hugh Cooper Administrative Center Work Shift: Weekday Schedule Monday-Friday (United States of America)Responsibilities:The Population Health Digital Outreach Manager is responsible for designing, executing, and optimizing digital engagement strategies to improve health outcomes and quality performance across Medicaid, Medicare, D-SNP, and Commercial populations. Working within an integrated delivery system, this role partners across the health plan, care management, quality, and provider operations to ensure outreach is coordinated, data-driven, and aligned with clinical workflows and value-based care goals. This position supports both current-state campaign execution and future-state capabilities, including population health use cases for future-state CRM implementation.Some key responsibilities include:Reports to: Director of Population Health EngagementDirect Reports: NoneKey Role ObjectivesDrive staffing efficiencies by transitioning appropriate outreach from manual phone calls to digital channels (SMS, email, portal, IVR)Increase member participation in clinical, preventive, and care management programs through targeted digital engagementSupport scalable outreach models that reduce operational burden while maintaining or improving outcomesKey ResponsibilitiesEnsure vendor-supported digital communications align with PHP strategyOwn end-to-end population health digital outreach campaigns across SMS, email, IVR, and patient portal channelsPartner with the Analytics Organization to translate targeting outputs into outreach executionAlign outreach strategies with STARS, HEDIS, and state quality measuresCoordinate with care management and provider teams to align outreach with clinical workflowsDeploy campaigns across multiple tools while ensuring consistency and complianceSupport implementation and operationalization of population health CRM capabilitiesParticipate in requirements gathering, vendor evaluation, and user acceptance testingMonitor campaign performance and drive continuous improvementEnsure compliance with HIPAA, CMS, and communication regulationsQualifications:Bachelor’s degree or equivalent experience in healthcare, public health, or related fieldExperience in population health, healthcare operations, or digital engagementHands-on experience executing outreach campaigns across digital platformsKnowledge of value-based care, STARS, HEDIS, and additional quality programsStrong collaboration and communication skills across multidisciplinary teamsPreferred QualificationsExperience with CRM platforms (e.g., Salesforce Health Cloud) preferred but not requiredExperience supporting implementation of new engagement toolsFamiliarity with Epic, Genesys, Jiva, or similar EHR/Telephony/Care Management systemsExperience with Tableau, Power BI, or similar toolsSuccess MeasuresReduction in reliance on phone-based outreach through increased digital adoptionIncreased enrollment and engagement in clinical and care management programsImprovement in STARS and HEDIS measuresCare gap closure ratesMember engagement metricsContribution to value-based care outcomesAll benefits-eligible Presbyterian employees receive a comprehensive benefits package that includes medical, dental, vision, short-term and long-term disability, group term life insurance and other optional voluntary benefits.WellnessPresbyterian's Employee Wellness rewards program is designed to provide you with engaging opportunities to enhance your health and activate your well-being.
