Physical Therapist (PT) - PRN at SWAN Outpatient Neurorehab in Phoenix, AZ Rehab Without Walls Neuro Rehabilitation Overview Provide 1:1 patient care in 60‑minute treatment sessions, focusing on functional rehabilitation for adults with neurological injuries. Collaborate closely with an interdisciplinary team of therapists to focus on patient‑centered outcomes. Provide personalized care in an innovative outpatient setting utilizing the latest technology and equipment including Bioness BITS, FES, biVABA, Assistive Technology, and more. What You Will Receive (PRN) Flexible hours – make your own schedule No evenings, weekends, or holidays Clinical autonomy Supportive team and leadership Professional growth & education opportunities Responsibilities Perform evaluations and develop treatment plans Regularly re‑assess effectiveness of treatment plans, attend staffing meetings and family conferences Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources Communicate patient needs and progress to the treatment team, physician, and family members Communicate with other disciplines to ensure collaboration, coordination of care, and enhance patient outcomes Qualifications Minimum of bachelor’s degree in Physical Therapy from an accredited program Current, unrestricted license as Physical Therapist in the state of practice Current Basic Life Support or Cardiopulmonary Resuscitation certification in accordance with state regulations A minimum of one year’s work experience as a physical therapist preferred Demonstrates knowledge of rehabilitation techniques related to complex neurological injury Communicates effectively and professionally in verbal and written interactions Ability to lift 50 pounds and move, lift, or transfer patients up to 100 pounds following safety procedures Proficient fine‑motor skills, visual acuity, and ability to walk/stand for extended periods Comfortable with pushing, pulling, bending, stooping, reaching, kneeling, and positioning #J-18808-Ljbffr Rehab Without Walls® Neuro Rehabilitation
