This job assumes responsibility for coordinating Clinical Pharmacy Services at a Long Term Care Facility. This role facilitates the development, implementation, and maintenance of systems that promote desirable patient outcomes through the use of safe, cost‑effective drug therapies. The position provides clinical consultation and clarification to practitioners, offers training and education, conducts competency assessments of pharmacists and clinical rotation students/residents (if applicable), and reviews the performance and documentation of clinical activities of all pharmacists. This position also serves as Pharmacist in Charge. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Contents are subject to change at the company's discretion. Education Required – Doctor of Pharmacy (Pharm.D.) Preferred – Post‑doctoral residency or fellowship Work Experience Required – 3 to 5 years of clinical practice Preferred – Long Term Acute Care Experience Experience in a supervisory role Certifications Required – Current Pharmacy license in the state of practice Knowledge, Skills, and Abilities (KSAs) Interpersonal skills necessary to collaborate with all members of the health‑care team, including physicians, administrators, nurses, and support staff. Computer skills for data entry and retrieval of patient information, proficiency with Windows‑style applications and keyboard use. Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals throughout the patient‑care process. Strong leadership skills. Job Duties Coordinates Clinical Pharmacy Services Educates staff on pharmaceutical issues Supervises, trains and develops department staff Provides patient assessment and care Maintains professional certifications, licenses and technical knowledge Adapts behavior to the specific patient population – respecting privacy, adjusting communication style, obtaining permissions, etc. Other related duties as required. Physical and Environmental Demands Medium work – requires occasionally exerting 20 to 50 pounds of force and/or frequently 10 to 25 pounds of force; may involve moving objects with more or less negligible force. Physical demand requirements exceed those for sedentary work. This job requires walking or standing to a significant degree and may involve pushing or pulling of materials or equipment. Mechanical lifting devices or team lifts should be used where appropriate. Must be able to stand for prolonged periods of time. Normal routine involves no exposure to blood, body fluid, or tissue, but exposure or potential exposure may occur. The incumbent works with patients who may have known or suspected communicable diseases and may enter isolation rooms, carrying an occupational risk for exposure to all communicable diseases. The incumbent may encounter hazardous medications and hazardous waste, and may be exposed to hazardous medications or waste within the environment through receipt, transport, storage, preparation, dispensing, administration, cleaning, and disposal. Risk levels may increase depending on essential job duties.
