Clinical Pharmacist Practitioner Murphy/Robbinsville Split

Job Summary The clinical pharmacist practitioner is a licensed professional with knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics. This clinical pharmacist is responsible for interpreting and evaluating appropriateness of pharmacological therapy to ensure that patients receive optimal drug therapy. This CPP (Clinical Pharmacist Practitioner) will be responsible for embedding in primary care clinics and carrying their own patient panel while also contributing in collaborative care. This position will be located in far western NC and will split time between our Robbinsville and Murphy clinics which are 45 minutes from one another. Both locations are 110 to 120 minutes west of Asheville. Key Accountabilities Pharmacist Responsibilities Select appropriate drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics Complete patient‑specific monitoring for efficacy, adverse effects, and clinical outcomes Communicate relevant findings and/or recommendations to other health care providers verbally and in writing Provide medication and device counseling to patients and caregivers Complete medical history and medication reconciliations Clinical Provide concise, applicable, comprehensive, and timely responses to requests for drug information Educate health care professionals, students, patients, and the public regarding rational drug therapy Maintain basic knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics in the area of practice Education Provide concise, applicable, comprehensive, and timely responses to requests for drug information Educate health care professionals, students, patients, and the public regarding rational drug therapy Maintain basic knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics in the area of practice Value‑Based Care Perform Medicare Annual Wellness Visits which will include, but is not limited to, medication reconciliation, laboratory reviews, medication appropriateness, immunizations required, preventative health screenings required, and address any questions that the patient may have about their care Contribute to quality improvement efforts and develop new initiatives, policies, and procedures Complete Chronic Care Management consults Administrative Establish and implement programs that help ensure cost‑effective drug therapy and successful patient outcomes Manage continuous glucose monitor initiative Implement policies, practices, and procedures for clinical pharmacy services, ensuring compliance with regulatory standards Collaborate with Director of Pharmacy to ensure optimal allocation of 340b resources Requirements Education / Experience Education: Doctor of Pharmacy degree from an accredited School/College of Pharmacy Experience: post‑graduate year one residency or equivalent experience in ambulatory care or rural health Licensure: current in‑good‑standing or eligibility for NC pharmacist license and clinical pharmacist practitioner license Other: NC driver's license for between clinic driving as necessary Skills Skill in exercising a high degree of initiative, judgment, discretion and decision‑making to achieve the health center's mission. Skill in establishing and maintaining effective working relationships with employees, policy‑making bodies, third‑party payers, patients and the public. Skill in organizing work, delegating and achieving goals and objectives. Excellent interpersonal skills and empathy towards patients, as well as strong communication skills, critical thinking skills, the ability to handle stressful situations, the capacity to function independently and the ability to document meticulously. Must have excellent process improvement skills and ability to understand clinic functions and department interactions. Intermediate ability with Microsoft office with emphasis on Excel and Word. Abilities Ability to identify trends and motivate workforce toward changes needed to adapt and remain competitive Ability to create and interpret reports from the practice management system. Ability to identify and implement opportunities for improvement and change. Ability to manage and supervise various positions and relate well to people from diverse ethnic and cultural backgrounds, as well as have a passion for working with at‑risk, culturally diverse populations. Ability to read, understand and interpret financial statements, spreadsheets and legal documents. Ability to develop and nurture vendor relationships for purchasing optimization. Ability to motivate diverse groups of employees to accomplish the goals and objectives of the organization Ability to hold subordinates accountable for organizational goals Ability to make difficult decisions Ability to adapt process improvement in accordance with organization objectives. Willingness to work flexible hours in order to meet the organization's needs/demands. Physical Requirements Seeing: Must be able to read written and printed material, as well as use computer, fax machine, Xerox machine and other office equipment. Hearing: Must be able to hear well enough to communicate with coworkers and others. Good hearing is necessary to receive detailed information through oral communication and to make fine discriminations in sound. Sitting: Must be able to sit for the majority of the work shift while answering telephone and communicating with others. Standing/Walking/Mobility: Must be able to stand intermittently and be capable of walking on hardwood and linoleum floors with concrete underpayments. Must be able to walk between various clinical areas and departments. Fingering/Grasping/Feeling: This position requires manual and finger dexterity and eye‑hand coordination for easy and skillful use of hands when working with and performing normal office duties and responsibilities. Driving long distances regularly to far western clinics. Work Environment This position is posted as a split location role where the Clinical Pharmacist will be spending days in both Robbinsville and Murphy NC, where we have clinics. These locations are 45 minutes apart from one another. Rural Appalachian Communities Low patient health literacy Fast paced work environment.
